I was recommended to Steve Kirby by my daughter (Get fit stay fit Norfolk) after 10 years of serious discomfort, Steve is a very nice guy and very well qualified for the service he provides which is fixing Neuromusculoskeletal Injuries. I contacted Steve as I was having issues after having to endure numerous operations on my hips, I have now had to have eight full hip replacements over this period, due to many complications. These complications are now behind me thankfully, but some quite serious discomfort remained with me. Including blood flow issues, nerve damage and neuropathy leaving me with a horrible numbness in my feet with tingling, pins and needles type discomfort. Recently whilst in conversation with experienced Health Care professionals , we discussed the subject of rehabilitation from injury and surgery . “Joint and Muscular Injury is not all about the, Recovery and Rehabilitation,” this may seem a strange statement from me , as a lot of my patients will state, to them I am all about the wellbeing, the recovery, rehabilitation and helping them get back to being the best that they can be.

This means the functions, that go on in your body aided by nerve and blood function to your muscles the brain, that affect your Proprioception, you may find an inability to move smoothly or as I explain to my patients ” They are unable to have Pain Free Fluid Movement” muscles shorten (tighten) so less flexibility , this can cause problems while sleeping or in a lot of cases lack of sleeping , pain and stiffness on waking making it difficult to get moving in the mornings, getting in and out of chairs putting on shoes, all these things are because the muscles are not supple , the need to keep muscles supple is clear. If muscles are supple then you have a better range of motion, less pain and better function, by keeping muscles supple it makes for a better outcome for surgery, a better outcome for surgery means a better outcome for recovery so it is a Win Win situation. You have to look after you while waiting for surgery.
Physiological
How will this present itself to you ?
What is physiologic stress? Physiological stress can be defined as any external or internal condition that challenges the homeostasis of a cell or an organism. in this case you, It can be divided into three different aspects: environmental stress, intrinsic developmental stress, and aging.
You may find your heart rate will increase, for a short and initial reaction, you may find swelling around the injured area, you may have a sharp stabbing, burning or tingling pain, this being nerve related . Movement may become restricted , also discoloration of skin in the area e.g. . Bruising, this is the first part, then it is about the after affects, the injury may need surgery, while waiting for surgery you may need to have medication, this has to be considered, though you have to work with your body to keep healthy, you may have an injury that causes difficulty in getting around, so you have to make changes in how you do things, we also have to consider aging, some injuries can have a worse affect the older you are , the need to keep bones in good order and ladies be aware after menopause bone density , is just as important as muscles being kept supple, we all need to try to age well, injuries in your youth can come back to haunt you as you get older.
Psychological.
How will this present itself to you ?
When I first, started to write this section, I thought about how people react to an injury, at first and it is basically, The Fight Flight or Freeze reaction, the adrenalin kicks in and we go onto Auto mode, then the realisation kicks in once we know the outcome of the injury, from the acute stage to the chronic stage it will all take a toll on you in so many ways because of the inability to do what you were able to do before , there is doubt, there is fear, also guilt, this can all lead to depression.
By now you may be wondering where do I go from here and the impact of extended time , waiting for treatment , in the form of NHS surgery, your muscle still need to work, but if you are not giving them chance, then the longer term issues can come into play.
Forced movement may rupture muscles, thus creating further issues, during that waiting time the injury can get worse the pain can escalate the painkilling medication is no longer effective so may be increased, while waiting for surgery you still need to stay active, but mobility becomes excessively difficult, you may end up or stronger medication that comes with it’s own side effects , this is when you may feel like it is all spiralling downward and out of your control, you have to be able to function for your own mental and physical health therefore,
Pain Free Movement is the Goal!
“I know it all sounds a bit doom and gloom, but that need not be the case, the need to keep supple to keep the endorphins working especially the, Beta endorphin (this is a great hormone we all love this hormone even if we don’t know it’s name if you are a runner this is the hormone that gives you that great rush, I know a lot of runners that say running becomes addictive because of that feeling they get, well this is the hormone that gives you that feeling, it is a endogenous opioid and a peptide hormone , it is produced in certain neurons within both the Central and Peripheral Nervous System it helps in themodulation of pain perception, both in the central nervous system and the peripheral nervous system. When pain is perceived, within the pain receptors, ( Nociceptors.)”
The Beta Endorphin main job is to decrease bodily stress and maintaining homeostasis resulting in pain management, it also plays a main part in the reward effects, and behavioural stability.
This is why I use the Bioneuro Sigma Q in my treatment of patients it is able to work through the Central and Peripheral Nervous System working on all three types of receptors that regulate the neurotransmission of pain signals.

Some of you may know that I have recently had to have major surgery, a bit of a traumatic time, in both mine and Jacqui’s life , we are grateful to all our family and friends, that have been with us through all this and for the Doctors and Nurses that were there for me .
Being the patient rather than the therapist, helping others recover has been odd, as any of my therapist fiends will tell you we are not the best of patients ourselves, I would like to share some of my journey with you in the hope that it can be of help to others.
I have been using the Sigma Q technology in my practice for over thirteen years, from early day to where it is today.
During all this time I have witnessed excellent clinical outcomes, patients lifted out of chronic pain from long term trauma related injuries,

Being a Neuromuscular Injury Therapist for 30 years, I have always had the strong desire to study, to achieve greater knowledge of anatomy, physiology and neurology.
I have always felt the need to further my skills, so I may enhance good clinical interventions and outcomes for my patients.

Looking at my patient bank that has mounted over the years, evidence is what you can see and what you know to be true , people want quality of life , Bioneuro Sigma Q technology is vital in my book for society in general, training, learning is continuous and it drives me to achieve .
Some months ago I was diagnosed with a cancerous tumour on my right kidney, at this stage I would like to thank once again the surgeons, nurses and staff at the Norfolk and Norwich Hospital, first for finding it and acting on it so quickly and the care I was given while being treated there, I underwent excessively invasive surgery and had 20% of my right kidney removed.
Following surgery I was given morphine for pain relief as it seemed at the time that it was intense and horrific. I had Robotic surgery, although it is more precise, the bruising was considerable, when I was able to start my slow and steady rehabilitation program, I started to get back to enjoying the benefits of The Bioneuro.
To aid with forming of the deep scar tissue that affects the Fascia, although the outer scars are still very evident , the inner scar tissue is pliable, healing has had it’s own related issues and the journey has had its ups and downs, trigger points have been an evident problem to deal with.
As the muscles have been recovering the multifidus, psoas and minimal gluteal muscles have been the to say the least uncooperative in their functional ability at times , thankfully I have been in the position, with the help form Jacqui and using the Bioneuro Sigma Q to deal with them effectively.
The positive news is, I am cancer free they managed to remove all the tumor as it was what they called Encapsulated so it had not had any chance to spread.
I am not on painkillers, I have improved pain free movement, able to work and look after my patients, I am a an Injury Therapist, it is my passion and I love my work, the human bodies ability to heal itself with the right sort of help has been more of an eye opener for me alter becoming the patient myself needing the aid of the Bioneuro Sigma Q , in combination with manipulation and massage.

So what better way than to strip out the Treatment Room give it a deep clean, walls, ceiling, floor, if it was there it was cleaned, then a face lift new cabinets and painting bookshelves . things we could not do while we were busy.

It was hard work, as the standards Broadland Council set are very high, but it is worth it we will keep up these standards knowing we will be keeping our clients safe while they are in our care.
Making everything COVID SAFE was the most important thing so our clients could come to us with the confidence and knowledge, that we were taking all the steps we could to protect them.
It was logical to work with Broadland council to make sure that we were getting it right.

We are pleased to say we have passed our Broadland Council Covid Safety Checks with Flying Colours and Awarded our Covid Safety Certificate .
During this last year we know how hard it has been for some of our clients, we do understand the difficulties, even more so we empathise with them.
Steve had to go into hospital for surgery himself, understanding how surgery affects a person physically, physiologically and psychologically are subjects we will be talking about, along with the need for the right support and care to have a better recovery outcome.
As a therapist I have been in the business since my 20’s things have changed over the years, learning to adapt and use the knowledge that I had gained with the treatment of each and every patient I grew in confidence, taking lots of courses, but I will say only the ones that I considered were right for the direction I was going and that would truly benefit myself and my business.
I found that my thirst for knowledge was going to increase, for me in 2007 things changed for me and I had a few decisions to make, one was to build my business to the point that I could be solely committed to my business and of course my patients, for me it was a no brainier, but to do this I had to be taken seriously by other medical professionals.
Knowing the anatomy, the muscles the nerves how the system’s work together, I had been given a couple of books while in my 20’s by a radiologist, to read, those books set me on my path, they were by Andrew Biel The Trail Guide to The Body and Muscle Energy Techniques Leon Chaitow, Leon became my hero, I bought my own copies of those book and still have them today and yes I still use it, I got to meet Leon, now they say you should never meet your heroes as you will be disappointed, but that was not the case, he inspired me even more , I then had the opportunity to take a training course with John Sharkey a fantastic anatomist and Leon, that was time and money on a course that were the best and well spent , I thank John for taking me under his wing and all his valued information, any of you that are considering taking a course John is the person to go to, it will truly open your eyes .
For me and how I treat my patients has always been, the holistic treatment of a patient, the path and affect of an injury on a patient, the network of soft tissue wrapped and protected by Fascia, how the breakdown of this incredible material can affect people how it can transport the messages around the body, how it interacts and it’s Biotensegrity ,This is the structural principle that describes a relationship between every part of the organism and the mechanical system that integrates it into a complete functional unit.”
How the body adapts, all these things count and it made me question about how It could affect a patients recovery , in 2009 I bought my first Bioneuro Sigma Q and along with the tools such as the roller and the ultrasound additions I have found it a useful tool, once again I found I had to adapt and change how I worked, I could no longer use mainly massage, it was to much of a scatter gun approach, I had to change my techniques and adapt them to get the best out of the Sigma Q technology , I found I had to use a more targeted approach and use massage at the end of treatment to relax and recover for a patient, I had to use more Neuromuscular pin point targeted techniques.

Working to aid the Efferent and Afferent Neurons
Efferent Motor Neurons will be promoted in their journey towards the muscle, therefore allowing better and increased muscle movement and function, while the Afferent neurons these are our sensory neurons, will be promoted in their journey to carry nerve the impulses from sensory stimuli towards our central nervous system and to the brain,
While the patient is recovering from a trauma injury, the fascinating process of Phagocytosis takes place , again with the use of Sigma Q technology the advantage becomes clear as it enables this process to take place faster giving the patient quicker ability to heal, more so than would take place with manual therapy intervention alone .
In sports therapy getting into deep tissue is difficult and a lot of muscles are hard to manipulate, especially when your working on a elite spots person, we know there are aids to help us out there, I use a Neuromuscular activation system, to give me an advantage, but the need to really follow that injury pathway is not always open to us, that track and how things have got held up, to unravel the damage that has been caused, you need a guide through it.
Our body provides that guide we just need to know how to follow it how to use it and how to make it count and most of all how to enhance that connection between our hands that are our greatest tools, to our guide, with out getting distracted by all the other issues and mixed messages that happen when a person has an injury.
I have spoken in the past and more so just recently, in regard to one of my patients Ivan his injury, how he has been improving and the work we have been doing to enable this .
I will say it has at times been hard work for both of us, but we have had one Goal to get Ivan walking again.
I have used what resources, I have had at my disposal along with a Neuromuscular Activation system, I have been able to use Neuromuscular Techniques, that I had been taught while on a Neuromuscular Course with The Great John Sharkey, at the National Training Center in Dublin

Having had a traumatic injury, I had evolved a technique of getting around, to which I used my Right leg as a crutch. In response to this my muscles became stiffer, and stiffer, I didn’t feel that conventional medicine was resolving my situation, I stated looking for an alternative being under a lot of stress due to my injury, I visited Chiropractors and other soft tissue technicians in the hope of providing some relief , but they couldn’t resolve the problem that my right foot was 2 inches off the ground and very reluctant to follow through in front of my left.
They had however, identified deep lateral hip rotator muscles as the problem, as they had totally atrophied
Finally a friend suggested I contact Steve Kirby of Getfitstayfit Norfolk, when Steve first examined me I felt he had an enormous understanding in this field, after my first visit I felt a release from what had seemed to be pulling me downward.

Here at our Neuromuscular and Skeletal Injury Clinic we treat patients with both chronic and acute injuries, caused by either sudden mild muscular trauma, repetitive muscular injury or in quite a few cases a long term muscular injury due to a very traumatic related incident.
I have always believed that when you treat a patient you are treating the whole of the patient, to do that you have to ask questions and listen to the answers give them time to think about and discuss with you how they are feeling, what had happened, how they are adapting what has being going on since the incident.
You need all the information and it is amazing how much people do not tell you at first, but as time passes more is revealed to help you put things into perspective.
This is what has been happening with one of our Long term patients .
Adrian presented at our clinic in the summer of 2019.
Blinded with fear of the future due to the view and consensus, it was possible, that he was suffering from Cerebellar ataxia (Cerebellar ataxia can occur as a result of many diseases and may present with symptoms of an inability to coordinate balance, gait, extremity and eye movement). This man used to swim for his county ,be very fit and had quality of life .
Knowing that specialised treatment with ( Transcranial direct current stimulation TDCS) was being trialled I felt strongly we could help with Bioneuro Sigma Q treatment .
Further investigations found Adrian had also suffered a major trauma, in which he had been being kicked in the hip by a race horse, being kicked by a horse is like being hit by a ton weight moving at about 20 miles an hour, this in anyone’s book is major impact injury, then stamping on his foot, let’s not kid ourselves here, we are not just talking shattered bones, but nerve and muscle damage all of a serious degree in truth it could kill you.
For Adrian it had caused dramatic physical deterioration, The impact for him being kicked by the race horse had caused Femoral Acetabular Impingement (FAI ).
Then to add to that, he then had to have abdominal prostate surgical intervention, this also caused numerous issues, his core stability was by no means in anyway in a good condition, leaving him with, very poor coordinated motions of his hip ,lower spinal column and femur life for this man was not great.

A Walk (Totally unaided down an Aisle) , Adrian said to me when we first met this was one of his main hopes he wanted walk his future wife Haley, down the aisle at their Wedding, to a future together building their lives again, then back to Work ,to return to his job, this can now be looked at and seriously considered.
That’s the future clinical outcome that I want to work towards for him, with the aid of Bioneuro Sigma Q, the same as I want for all my patients ,so they can have quality of life to the full.
Folks I have read a lot of comments in reference to Sports Massage Therapist, or Soft Tissue Therapist not being able to open just yet .
In all my 30 years of being a therapist I have never known so many of my colleague’s to be so despondent and under valued and it saddens me, because I know how much work they do.
As the re-opening is starting after Covid 19 ,a Soft Tissue Therapist said “If we are waiting for them to say the words Sports Massage or Soft Tissue Therapist can re-open , then forget it they won’t “ .
The reason being they NEVER said that Sports Massage or Soft Tissue Therapist should close, please bear with me on this , as I would like you to think back to 2012 and the Olympic Games in London, when a considerable number of Massage Therapist and such were helping out by looking after the athletes, at the end they were thanked for their work.
in fact I do think it was, Boris that thanked them, but what did he call them ? “It was Physio’s” he like most event organisers and a lot of athletes, just think of all therapist, that are looking after them as Physio’s.
They do not concern themselves with all the different organisations that are involved, they just want someone to do the job, to help fix them when they are injured.
After all the term Physiotherapist according to the Oxford dictionary is “A person qualified to treat disease, injury, or deformity by physical methods such as massage, heat treatment, and exercise, be it after a hip replacement or injury”, there is no definition for either a Soft Tissue Therapist or Sports Massage Therapist in the dictionary, so no wonder they get confused as to what we do .
It is our own bureaucracy that has created the divisions and the discrimination, we have one word that causes us problems and that word is, MASSAGE it is not considered by some insurance companies as a treatment, even if you are dealing with rehabilitation massage after injury, they consider massage as a preventative just for relaxing, they do not consider the beneficial out comes for a client .
The word holds so many connotations, “a massage” is considered a source of pleasure, ” A relaxing massage” we have all heard the term, we also know that when you add the word PARLOUR, then bells ring in any professional therapist head.
As it has very seedy connotations and a word we need to stop using to define our business’s, as the definition in Collins Dictionary states “A Massage Parlour is a place where people go and pay for a massage, some places that are called Massage Parlours, are in fact places where people pay to have sex.”
As Boris is the type of person that would and does very often refer to the Dictionary Meaning, is it possible that therapist are asking the wrong question ? First should we ask “What sort of establishment is Boris referring to when he states Massage Parlours, are to remain closed, does he know the difference between Physiotherapists and Soft Tissue Therapist.?
We may find that neither he, or his other party ministers understand the difference, and that we have been categorising ourselves unnecessarily, because of a word.
Please do not misunderstand me, I have agreed and adhered to the rules, but I do feel that this is now a matter of confusion, because of how we define our businesses.
I am not a Massage Therapist, I use massage as part of the treatment process for my patients, I am a Neuromuscular Injury Therapist and I have worked hard to achieve this.
I work with and Consultants, GPs Neurologist, Orthopaedic Consultants, I have had the privilege of being trained by an incredible Anatomist , I am able to discuss and receive advice in what I am achieving for my patients, with some very well respected consultants , I deal with injuries and long term rehabilitation, I have worked with people across the social spectrum from elite athletes to elderly folk, that have had hip or knee surgery and find they struggled with the exercises given to them by the physiotherapist and they need a more hands on approach and support, People Do Not come to me, for a Relaxing Massage or a Happy Ending, I do not run that kind of establishment, I treat patients that need my help with trauma related pathologies and injury prevention.
Over the past few months the vital need to show good clinical outcomes has become more important than ever to lift people up .
“We first met Ivan at the end of 2014 . He presented with a number of chronic trauma pathologies which had occurred in 2003″
He lives quite a distance away, he had continued to work and sail with his injuries, unable to come for regular treatment having quite a few other commitments, but when he did manage to get a clear week, so he could come for treatment, we made the most of it and it truly has been paying off, yes it has been a slow and steady journey.
He presented initially with chronic hip ,lower back pathologies, his foot was rotated out of line and his right leg artificially shortened so it was not much more than a crutch, a number of muscles had atrophied greatly (wasted nearly away ) in his Gluteal and Lateral Rotator’s muscles his Gluteus Maximus had atrophied so much, to the point that his hip bone was very evident and in truth he had no buttock on his right hand side, Deeper lateral Rotators had practically seized and were atrophied, Spinal Rotator muscles and their ability to function correctly were very inhibited, with no functional lateral Rotator, his ability to walk (gait) was excessively limited .
His upper torso had pulled over to the right hand side to about 75 degrees, quality of life was very limited .
This man loved and still loves sailing and yacht racing on an international level, to which he has a great deal of knowledge and also about boat building his other passion.
Ivan had received numerous other interventions prior to attending our clinic with no positive constructiveness in their outcomes
Working very hard on these muscle and nerve pathologies, with the use of a Neuromuscular Activation system, sending frequency deep into the body to activate nerve messengers, Motor Neurons and Neuropeptides, has aided greatly to enhance my ability to target strategic nerves to help restore semi morbid fascia, therefore helping to achieve a very positive clinical outcome, allowing activation of the deep nerve pathways, therefore allowing much greater muscle function ability, this is not something that could be achieved with manual therapy alone.
Ivan had a solid determination to achieve, so that when we were not able to have treatment sessions, he kept up with the exercise plan, that between us we had decided was the best way for him to go.
Along with Ivan’s Physiological factors that have improved greatly and he is able to achieve a great deal more and will continue to do so. Psychologically, Ivan feels more confident and assured in his ability to move forward with his life.

He can stand up straight without dragging his right leg behind him , his posture has improved greatly, we have activated very deep muscle groups in his body, to achieve this clinical outcome so far.
