Whip Lash Injuries

Whip lash is a sudden jolt or jerk to the head , in a car related accident !. What does a sudden jerk do??. Pain and shock throughout the body. Shocked skeletal muscle equals Whiplash nerve related injuries. Every muscle in the human body requires a nerve and good motor neuron supply to function correctly. Muscle and Fascia withy out correct amount of blood or nerve supply will atrophy /die / become solid !.

The vital importance for the Therapist to use Subjective questions/history taking and physical assessment is crucial to have a good clinical outcome. Lack of ability to move in all planes need to be fully noted. Discussions, and treatment plans and then treatment intervention.
Modalities for treatment. People ask , why I use the Sigma Q technology dry system modality. Being able to penetrate through fascia/skin non evasively. The system has an variable dial which can be used to apply /sound waves /frequency to areas of injury and trauma aiding nerve activation. Being able to locate the excite Trigger Point/ Area of pain with the “Roller” and also very powerful Ultrasound can be applied. The other attraction of using Sigma Q is the therapist can massage or manipulate whilst the Sigma Q is in operation .

Chronic Whip lash, life altering, full of medication .Restricted movement with nerve entrapment.

Recently a patient attended my clinic and presented with chronic whip lash.

First Treatment Session. Following a full assessment, treatment was commenced. Using the roller, the patient had suffered from severe subcostal pain. Locating Trigger Points TRVP’s on T12/ Quadratus Lumborum QL / Subcostal nerve and L1 working the Ilionguinal nerve. Finishing the first session with massage and manipulation to Sternocleidomastoids SCM bilaterally also the Scalene Triangle bilaterally. The PSIS Posterior Superior Iliac Spine was also put into the correct position.

Feed back from patient. Able to breath much easier and deeper. Physically bend to the side bilaterally. Not feeling need for pharmaceutical intervention to a degree.

Second Treatment Session

Worked on LM Lumber Multifidus and bilateral ITB’s Iliotibial Bands and TFL’s Tensor Fascia Latae ‘s. Being able to let the intervention on the Lumbar Multifidus with Sigma Q you could identity nerve root damage and allow the frequency to treat the damaged location. Then worked on TFL’s and ITB’s. Ended treatment with massage and manipulations. Working on the Spinal Rotator muscle group

Feed back from patient.

Felt more able to walk pain free, enjoy walking her dog. Able to go shopping and push a shopping trolley. Further lack of need to take strong pharma logical intervention.

Third Treatment Session

Worked on upper Thoracic Region. Mainly focused on TRVP’s. All active, latent and referred, using ultrasound and sigma Q intervention and massage and manipulation.

Feedback from patient. Further greater ability to sleep without medication, ability to participate in dog agility class. Able to participate in return to work programme.

Fourth Treatment Session

Worked with Feet on mats and allowing the frequency to work into the right calf where the patient who had some years previously fractured her Tibia. The nerves where activated .Better spinal rotation was evident. Then manually treated spinal rotation which had improved well in all planes. This was aided with ultrasound to the third Lumbar Vertebra aiding further ability to rotate free from pain.

Further manipulation to STL Sacrotuberous ligament.

Better spinal rotation and work for longer at desk up to 40 minutes with no pain or discomfort. Proprioception improved ,able to stand for longer periods. Much better pain free ability to move in all planes of movement. Also further lack of need for pain killing medication. Strong evidence of Improved Cranial Rhythmic Impulse with Improved sleeping and being able to coup with work demands.

Fifth Treatment Session

Further treatment session to back. No Trigger Points TRVP’s on right hand side. Sigma Q applied. Full massage and manipulations to legs with a number of MET’s (Muscle Energy Techniques)

Feed Back from Patient. Greater ability to stand painfree and doing spinal rotation without restrictive pain for a considerable time. Pain was muscular.

Steve has been an absolute miracle worker. I injured my back 15 years ago in an RTA and have had chronic pain since. January 2026 I then had acute back spasm which lasted on and off for over 4 weeks which was extremely debilitating and meant I was unable to work, drive and general functioning was extremely difficult and started to affect my mental health significantly. Following a call to Steve, he fitted me in that afternoon and immediate improvement and relief followed. For years my life was defined by pain

For years, my life was defined by pain. Not the kind that comes and goes, but the kind that follows you everywhere — into your workday, into your sleep, into your thoughts. I had problems in my ankle, knees, hip, back, neck, arm, scapula and shoulder. I had surgeries, injections, nerve blocks, endless appointments and even more waiting lists. Each time I hoped for relief, and each time I was told there was nothing more that could be done.

The pain was constant. The exhaustion was constant. And the dark thoughts that come from living like that were becoming far too familiar. I questioned how I was supposed to keep going when every day felt like a battle I was losing day by day.

My experience changed when I began treatment with Steve at Get Fit Stay Fit Norfolk and was introduced to Sigma Q.

I initially assumed Sigma Q was just another gimmick. After so many unsuccessful interventions, it was difficult to believe anything new could make a difference. Just before Christmas a patient came to see me and presented with a knee injury but further questions showed that 25yrs previously they had been involved in a serious RTC (Road Traffic Collision ) . A big question that at times we all might have to face. 

I believe preparation helps a positive outcome . My sepsis and major surgery history . December 2024 ,I was rushed into hospital, finding out I had sepsis ,severe diverticulitis needing major surgery having a Heartmans procedure. Stays in ITU for 18 days and then not discharged from hospital till February 2025. Told not to expect for 18months . Having a stoma was a shock to the system but I’ve got used to it . The way up is physical ,physiological and psychological . Well, now I am preparing for abdominal reconstruction surgery in summer 2025.

We will look at all my stories of hospital , past ,present and looking to the future. All aspects of preparation.

Horse Riding accident followed by trauma injury whilst walking her dog and twisting her ankle resulted in nerve damage.

The result was inability to balance and control the left leg. Coordination was also an issue for this very proactive lady who enjoys running and Tap Dancing . Doctors , physio and exercises for a prolonged period of time had no constructive clinical  improvement. Why was my patient’s question ?. Injury had caused sporting and recreational efforts to be very arduous for my patient. Rachel , saw and seized after the investigation of the benefits of sigma Q .  Sigma Q ,based on frequencies working in the nervous system  aiding motor neuron function . A full examination showed it was a nerve related problem with the Babinski sign being reactive. ( This showed a previous injury to the Spinal Cord). Thoughts were confirmed  following my examination. Treatment was started with  feet being placed on mats and frequency working into the ankles. Frequency working through the fascia in the chronic ankle pathology ,gripping the ankle with supportive ultrasound to clear scar tissue. Yes, the Babinski sign was very reactive when we started treatment but it has significantly been reduced with treatment with sigma Q.

Proprioception, balance and coordination have been improved greatly.

Fibromyalgia syndrome is known as a collection of symptoms . Fibromyalgia , an indiscriminate condition that attacks people ,research has shown more women than men . Pain of all descriptions. All things going wrong ,difficulty sleeping , memory not great able to think clearly, certainly not positive . Energy is very low at times as well.
Patients that have been in my clinic who have received a diagnosis of “Fibromyalgia ” tell me often they feel they are on the “Scrap Heap” of life with depression very evident . I don’t believe Fibromyalgia is a diagnosis, it is a symptom in my view . Very often ,it started with some form of trauma related injury . That is the place to start . I use the full range of diagnostic tests ,eg seeing range of movement / & restricted movement .

I also use a cutting edge piece of equipment, the “Sigma Q dry system ” . The attachment of the “roller” aids to locate the exact point of the trigger point in the body . Trigger points known as “active ” latent ” or “referred” points of pain . Trigger Points /hyperirritable points preventing good blood supply and nerve /motor neuron function .  Steve . “It hurts”. This  is the very frequent  comment  I hear when I have my first consultation with a patient . Especially with Knee pain .

What caused the pain to happen , examination carried out to assess the injury / pathology .

Why ,  the injury has occurred. Very often where the pain is the problem is not .  Muscular , nerve  and tendon can cause pain.

“Absolutely amazing. After being told the pain was in my head and going to the hospital and being told there was nothing wrong except a bruise dispiste struggling to weight bare Steve has managed to get my knee back to how it should be after being told multiple times it is a weak muscle. After a months later my knee went again at work and the GP would give me something for the pain but still saying there was nothing wrong. Then being referred to a a GP physio and then being told it wasn’t nerve damage and yet again there wasn’t anything wrong Steve took one look and knew what to do. After a 2 hour appointment I was able to sit comfortably, walk around with our feeling like I had a dead weight and can lift my knee higher than 5 cm. Could not ask for a better treatment he listened and found the problem straight away.”

I have always in my life tried to keep fit and healthy especially after having a Thyroid Cancer issue in my 20s, I truly thought that was my scariest time of my life , but the circumstances that affected me, that started on the 26th November 2024 have really changed my opinion.

I am Normally very fit and well , I trained twice a week, looking after my patients to get them back on track in their lives after injury and in need of rehabilitation.

I found I was on the receiving end of patient care, 26th November 2024, I started suffering abdominal pain, which became very acute, I found myself heading to A&E at the Norfolk & Norwich Hospital, all signs pointed towards my appendix , then blood tests, and a MRI changed that !!.

A young doctor ,that was heavily pregnant at the time saw me, I will admit I was worried about her she was obviously tired, but she was also very committed to her job she said . “Steve , the MRI shows you have a problem with you colon it looks like your staying with us !”, followed by .”We are looking sepsis as well”.

Being ,pushed to a ward ,many things cross your mind, I remember not having a great nights sleep ,doctors coming looking and discussing potential treatment with me all I wanted was to be out of pain.

The next morning I met the senior consultant, he felt that very strong antibiotics were going to save the situation .

During the next few days ,I felt very out of it, in every physical form imaginable, I could not eat and even drinking was a chore, but after a few days I felt slightly better then at the end of the week I was discharged.

Great I could get back to normal life, but that was short lived, within 24hrs, it was back to A&E found myself readmitted to hospital.

Further acute abdominal pain, a lot worse , this time ,surgeons and their teams very much on the scene . I was also having pain in my shoulders near a big muscle called “Rhomboids ” . Scans , MRI’s and more urgent blood tests .

A surgeon when he got the results ,rushed to see me . ” Steve , we have to operate on you urgently .”You have a Necrotic Gallbladder and your Colon requires surgery urgently !!!!!.

There are no words to describe that news , Fear , In trepidation, being told such news brings many feelings to your mind and body .

Yes , I have a faith . I am a Christian and believe firmly in a God who walks with us in the good and bad times . I will say it took all my faith that day and I asked my wife to ask people to pray, my other strong belief in God is not deaf or blind. He knows and see our needs , I am so Thankful he does

Being rushed to the Operating Theatre ,meeting the surgeon and his team . the procedure called a “Hartman’s “.

Being told , my chances of getting through it, that was the last thing until I woke up in ITU a number of days later .

Tubes coming out of my chest . Being pumped full of heavy drugs such as Ketamine, Morphine and Fentanyl I felt rather out of it .

I think having major surgery you can either sink or swim mentally, as I said I look after quite a lot of patients, I am very used to looking after others , but my being in ITU was certainly not on my radar.

Determination ,kicks in ,Sepsis ,major surgery takes it’s toll. this has ravaged me physically and mentally, losing two stone in weight , being unable to wash myself or lift a cup I just was not me and felt so weak.

“I wanted to get better and every day is a school day” I had to kick in to my grey matter.
You never stop learning about the human body and this was a opportunity for me to learn more about mine and what I needed to do to get back to being me again.

Watching , chatting to staff nursing, and medical you can pick up a great deal from them .
the frustration I had when I did not get answers ,drove me up the wall , as I have always said,
“Health is physical ,physiological and psychological”.

I knew I needed to Exercise but building up slowly and also reading for a while I felt like I had brain fog and could not keep a conversation going , let alone read a page in a book , I stated with my hands , I had no grip properly so it was difficult at first , but things started to come together , then the arms, I improvised with ,small plastic bottles with water in a weights, slowly building up by adding a bit more water , then when I was allowed out of bed what a shock I felt like I could not stand let alone walk.

It was a bit of a struggle, but I was determined to get there, so set small goals each day, I was shocked when I saw myself in a mirror how much weight I had lost and how ill I looked, the other thing was cold , even thought the heating was on , as anyone that goes into the hospital knows, I still felt cold ,at the same time craved fresh air.

By this time It was getting near Christmas I was then moved to another ward and I got a window so I got my fresh air but wrapped up, by this time I could walk to the door of the ward ,it did wear me out though .

I was discharged on New Years at the time there were concerns about Norovirus and I was vulnerable so was better off at home .

Unfortunately sepsis and every thing else can leave it’s mark and I have been back in hospital I had a pulmonary Embolism, again a bit of a shock and very scary.

I have been home since the 1st February 2025 slowly ,climbing that hill of rehabilitation, learning to adapt eating ,sleeping getting back into a sleep pattern was difficult flashbacks and some nightmares still linger along with the “What ifs, yes I question everything and still ask why me?” progressive exercise is working .
I still have a long distance to go to even get to be where I was.

Things still have to be assessed by the medical teams, there are still some issues sepsis does not let go that easy and yes I still have problems that have to be sorted out, I have adjusted to the changes in my body I no longer feel that it is not me .

“Do I want to go back to work as a Therapist ? ” of course the answer is yes I do ,but part time when I am fit and able.

A big lesson I learnt in hospital is “Life is precious you never expect things to go wrong, but they can and they will we just have to adjust and deal with them the best we can, and that we will get the best support that we can get , we need to be very thankful for the people to get things right, I am thankful for all my friends a family that have supported me and will continue to do so.

I know I am not a patient ,patient and I want to be back there for my patients, I will be back for them as soon as I am able .

Finding myself heading for A&E as I sustained a very serious injury – a spiral fracture of the humerus in my arm, along with a broken bone in my shoulder and numerous very livid bruises all over my body. My first thought was “I need to see Steve as soon as possible” however, back home and a visit to hospital here confirmed I would be in a body brace for the foreseeable future. Three months on and the body brace was replaced with a removable sling with a reassurance from the consultant that I could at last seek remedial therapy. Unsure as to whether Steve could actually do anything for me, I got in touch and made an appointment. An arm that had been restrained in one position 24/7 was not going to move easily but Steve took on the challenge. With his amazing Sigma Q technology he set to work. Now I am totally ignorant as to the ins and outs of how or why this form of therapy works, I only know from past experience that it does – like a miracle. Apparently, Steve told me, I was suffering with Neuropraxia and it was making my movements very restrictive. Steve set to work on locating the affected area and after an hour of treatment he invited me to stand up and raise my arm as far as I could. Unbelievably, I could raise my arm above my head. It was as if someone had unlocked a door inside my arm. Three days on and my arm is still mobile and pain free. I had been warned by the hospital to expect many months of discomfort and recovery; I can’t wait for my next consultant appointment to show her how wrong she was – my recovery time has been halved thanks to Steve and Sigma Q.

Earlier this year I was enjoying a new found freedom of movement thanks to Steve Kirby’s comprehensive Physio and massage together with Bio Nero ,Sigma Q treatment. Bio Neuro had previously found an old mid 80’s Back injury from hauling a boat out of the water. The Bio Neuro complimented by Ultrasound had helped repair the nervous stem on the Right side of my lower back improving my ability to walk and hopefully return to competitive sailing.
Back in the Winter/Early Spring period a Storm brought down a large tree necessitating assistance from myself. ‘Just can’t stay away from these jobs’ Net result was my ankle and left leg started to become painful at times with horribly reduced movement. Initial Bio Neuro, Sigma Q investigation revealed that it was hard to get the necessary signal penetration in an area around the ankle. The fact was that some 30years earlier I had turned my ankle over badly and just walked it better over a period of weeks. This had caught up with me. During Lock down lot’s of events were held online rather than physically and I felt it was better to use the time wisely and continue studying, I believe we all need to keep on moving forward and gaining knowledge to help our patients and keep up to date with what is happening.

To that end I have been in the happy position as a member, to be able to attend quite a few of The Royal Society of Medicine’s on line events, these give me invaluable access to learn from the best, to listen to some great speakers on some brilliant topics.

Under normal circumstances it would be a trip into London and a stay overnight , but this way it has meant I can enjoy the event, get to ask questions and have plenty of time to study the subject, a win, win situation.

The Shoulder is where the humerus (upper arm bone) meets the scapula (shoulder blade) It is a shallow joint to allow more flexibility of movement, but it also means it can be more prone to injury and dislocation along with nerve entrapment. This is a  joint injury that can be ,especially, devastating and totally debilitating, it can easily cause distribution to other structural and biological functions around it .

This is a  major area that  I have  found deeply interesting,  the discussions revolved around  non operative reconstruction ,the need to give a good prospective evaluation of the  trauma pathology. Examining how Neurovascular Injuries and Muscle Paralysis, can be treated with constructive intervention  to aid the quality of life for the patient .

These are two of the major areas, that are of interest to me.

Neurovascular Injuries

Neurovascular Injuries are damage to major blood vessels in the body with  further complications, with the injury being the  loss of sensation, tingling or pins and needles,  being apparent to the patient .

Muscle Paralysis 

Muscle Paralysis in simple terms means a loss of control and strength ability ,this  causes the nerve communication ability from the Spinal Column to be  compromised , there are four types of paralysis.

The Shoulder joint holds four major nerves ,  the origin of all these nerves originate from the Cervical Vertebrae C5-C7 .they are the:

Motor Neurons and Neuropeptides  carry nerve messages to help innervate muscles to work correctly, any injury or trauma will impair this function .
