Cortisone Injections are Not For Everyone

Brian, who is a very active mature gentleman, keeps fit with weight training and general house hold duties, suffered a shoulder injury nearly a month ago.

He was in great pain, his sleep was disturbed and everything was becoming very gruelling and hard just to get through the day, as his physical ability became more  impaired, at the time he was looking after his partner after she had just come out of hospital after having  had a hip replacement, The GP suggested a cortisone injection!.  A discussion with his partner took place.

Brian then came to see me. I Treated him on the Bioneuro “Dry” Therapy (∑Q®) system and Neuromuscular and Sports Injury Therapy, as I have found they work so well together to achieve better results for clients.

What is cortisone?
Cortisone is a type of steroid that is produced naturally by a gland in your body called the adrenal gland. Cortisone is released from the adrenal gland when your body is under stress. Natural cortisone is released into the blood stream and is relatively short-acting.

Injectable cortisone is synthetically produced and has many different trade names (e.g. Celestone, Kenalog, etc.), but is a close derivative of your body’s own product. The most significant differences are that synthetic cortisone is not injected into the blood stream, but into a particular area of inflammation. Also, the synthetic cortisone is designed to act more potently and for a longer period of time (days instead of minutes).

How does the cortisone injection help?
Cortisone is a powerful anti-inflammatory medication. Cortisone is not a pain relieving medication, it only treats the inflammation. 

When pain is decreased from cortisone it is because the inflammation is diminished. By injecting the cortisone into a particular area of inflammation, very high concentrations of the medication can be given while keeping potential side-effects to a minimum. Cortisone injections usually work within a few days and the effects can last up to several weeks.

Are there side effects? Yes. Probably the most common side effect is a ‘cortisone flare,’ a condition where the injected cortisone crystallizes and can cause a brief period of pain worse than before the shot.

This usually lasts a day or two and is best treated by icing the injected area. Another common side-effect is whitening of the skin where the injection is given.

This is only a concern in people with darker skin, and is not harmful, but patients should be aware of this.

Other Side effects of Cortisone Injections

Although rare, can be quite serious. The most concerning is infection, especially if the injection is given into a joint. The best prevention is careful injection technique, with sterilization of the skin using iodine and/or alcohol. Also, patients with diabetes may have a transient increase in their blood sugar which they should watch for closely.

Because cortisone is a naturally occurring substance, true allergic responses to the injected substance do not occur. However, it is possible to be allergic to other aspects of the injection, most commonly the betadine many physicians use to sterilize the skin.

Can I get a cortisone injection more than once?
Yes. There is  no rule as to how many cortisone injection can be given. Often physicians do not want to give more than three, but there is not really a specific limit to the number of shots.

However, there are some practical limitations. If a cortisone injection wears off quickly or does not help the problem, then repeating it may not be worthwhile. Also, animal studies have shown effects of weakening of tendons and softening of cartilage with cortisone injections.

Repeated cortisone injections multiply these effects and increase the risk of potential problems. This is the reason many physicians limit the number of injections they offer to a patient.

Hinged knee replacements for salvage date back to GUEPAR, but did not stand up to wear.

The history of knee replacement is the story of continued innovation to try to limit the problems of wear, loosening and loss of range of motion

The Guepar Valgus Hinged prosthesis was first used at Vancouver General Hospital in 1975 – In April of 1977 a local modification was introduced in an attempt to reduce the indecent of patellar subluxation.

The modification eliminated the several degree angles between the femoral and tibula components.

We have come a long way from those days and hips and knee replacements are now common place and recovery from such surgery is quite dramatic, from knowledge of patients the general consensus is it is a quicker recovery from a hip replacement than a knee replacement  and although hip replacement are mainly Titanium knees these days are  mostly a lighter carbon.

No matter how we have changed the design of the replacements and the speed of the recovery we still have the problem of pain, this is still something we have to overcome and people heal a lot quicker when they are not in so much pain and can start to move about.

Steve,

Marcus and I wanted to drop you a testimonial about our experiences with our treatment/therapy.

I have known Steve for over a year now and haven’t had to require your services – so to speak! However, the moment came about 1 month ago when I managed to pick up a horrid cold which led to me coughing and sneezing and unfortunately putting my back out.

The real worrying issue being I was due to go away for the weekend in London with Marcus and the pain was causing me to be in so much pain, even in bed laying still it would be excruciating.

I called Steve up on the Friday morning and he fitted me in 8pm that evening so I could have some therapy before going to London the next day.

Although Steve provides Sports Injury Therapy to clients – he was unable to massage me due to my cold – which would have moved the germ around my body and made me feel worse.

Whilst visiting Steve for treatment my husband Marcus has had a poorly neck for about 6-8 weeks following a whiplash injury.

Movement is much freer now and as a result he is able to move his neck with much more flexibility and very little stiffness/pain now. Once again, exercises to back up the treatment have certainly helped too.

Chris, a plumber of  had carried a serious shoulder injury for number of years during his working life.

He had searched all different avenues, to find relief and try and regain full physical ability again, for his shoulder.

The effects of a long term chronic physical pain brought special challenges to Chris in his working and social life.

Chronic rotator cuff tear begins with a small injury, which causes inflammation of the sacks which lubricate the shoulder joint (bursitis).

With repeated injury, this may progress to inflammation of the tendon component of the rotator cuff (rotator cuff tendinitis).

Eventually to a tear in the rotator cuff itself, which is associated with pain, decreased range of motion of the shoulder and arm, and resulting weakness and a withering away of the shoulder muscle, he had suffered from a neuropathology/deep somatic/nociceptive pain.

This had turned a memory of the pain being formed.

“This is was pain! Dull, aching, sickening poorly-localized pain on a daily basis.”

We had a phone call from Michael saying. ”Please can you help me I’ve got a pain in the neck and could do with your help “.  This was certainly a Slight understatement!  as we found out to our surprise a day or so later we met Michael.

There was this tall man, with his head on his left hand shoulder, a position he had be forced to get use too.

“His neck was locked in this position.”

 

Michael explained that 15yrs ago he had suffered a serious road accident, resulting in whiplash, a condition known as “Wryneck”. 

His GP had referred him for physiotherapy, but the treatment he received had no positive effect and he was discharged after a short period of time, he had suffered discomfort all these years.

Being a builder this also caused problems with physical abilities, he was not able to do all the work he needed to do and was in constant pain.

I was born with scoliosis and had a Spinal Fusion at three years old. The operation worked very well up until the age of around 20 when I started to notice lower back pain. This increased over the years leaving me struggling with chronic pain. I tried various things like epidural and nerve root injections but nothing seemed to work. In June 2016 I underwent surgery to correct the curve in my spine, taking a piece out and replacing it with a metal cage as well as two metal rods either side for support. Unfortunately during the operation The surgeon caught my spinal cord leaving me in Spinal Shock and temporally paralysed from the waist down, but within days I started being able to move my legs very slightly but I was unable to move my toes or my feet.

I’m a post lady and an active sports lady which I have never not once had a problem with my back.

To my surprise I woke up about a month ago with aches and pains where my muscles just went into a spasm.

I did not knows what to do so I went to the doctor, he recommended me to go and see a Sports therapist.

Sciatica

 As well as being a web developer, I’m also a Special Constable for my local force. A few years back I injured my back on duty, and went to see Steve Kirby of Getfitstayfit Norfolk about it.

Steve treated me with Neuromuscular Techniques Sports Injury and Sports  massages, but also with the Bioneuro Therapy System Sigma Q (∑Q)® .

This basically wakes up  your muscles and encourages them  into a workout as well as getting deeper than a normal massage can.

After I had been on this a few times I could feel the difference quite significantly!

Being sat at a computer most the day building websites, did start to cause a bit of a nuisance to my lower back again, especially the Latissimus Dorsi muscle which was where I caused the injury.

I decided on keeping active by going to the gym nearly everyday, sometimes before starting work, lunchtime or in the evenings.

As well as this I started personal training sessions (8 a month). Since taking on all this exercise, I have lost a lot of weight (12KG) and gained lots of strength and muscle, but in doing this I have caused more strain on my lower back again.

After I finished my training session on Thursday evening, I could certainly feel a twinge again in my lower back, and didn’t want to cause any more lower back issues or injure myself any more;

The previous night I had been out with friends clubbing, and I felt pain in my lower back as well as my hamstring feeling tight, which got worse during my personal training session. 

I was concerned about this I thought that I had to been seen to by my Sports Therapist and friend Steve Kirby. 

I made the 3 hour drive to Norfolk, “I live in Surrey” he put me on the system which really worked on my lower back, and I could really feel it working deep on the muscle, as well as this my hamstring was loosening up as it was really tight, and after the session I felt lighter on my feet and able to move more easily.
