What is Symphysis pubis dysfunction? How is SPD diagnosed?

SPD is caused when the gap in the pubic joint widens too far. The average gap between the bones in a non-pregnant woman is between 4mm and 5mm. During pregnancy it’s normal for this gap to widen by 2mm or 3mm. If the gap is 10mm or more, SPD is diagnosed. It’s rare, and can only be identified by scan or x ray.

What causes SPD?

The two halves of your pelvis are connected at the front by a stiff joint called the Symphysis pubis. This joint is strengthened by a dense network of tough, flexible tissues, called ligaments.

During pregnancy your body changes and you produce a combination of certain hormones, to help your baby pass through your pelvis as easily as possible, one such hormone is Relaxin this is the hormone that softens your ligaments. This then allows, your pelvic joints to move more during and just after pregnancy, but if the joint has become too wide, or for one side to move more than the other it creates the condition known as Symphysis Pubis Dysfunction (SPD) it will cause inflammation and the pubis to become tender, in some cases more severe pain.

Is the size of the gap the problem?

In truth no, the gap in your joint doesn’t bear any relation to the amount of pain you may feel. Many women with a normal-sized gap feel a lot of pain. It comes down to a lot of different things it seems one school of thought is that you may be more likely to develop SPD if you started your periods before you were 11, or if you are overweight .

Though we have treated a client who was far from overweight, and had not started her periods early, “So please don’t, beat yourself up or blame yourself for the condition there can be many reasons.”

When does SPD happen?

Unfortunately SPD can occur at any time during your pregnancy, or in some cases after giving birth. You may not have it on your first pregnancy you may notice it around the middle of your pregnancy. One thing is for sure that once you have had SPD in one pregnancy, it is more likely that you’ll have it next time you get pregnant Also the symptoms may also come on earlier and progress faster

What are the symptoms of SPD?

Pain will most likely be in the pubic area and groin, but you may also have the following signs, you may feel a grinding or clicking, sensation in your pubic area.

Pain can be down the inside of your thighs or between your legs. It can be made worse by parting your legs, walking, going up or down stairs or moving around in bed.

Back painpelvic girdle pain, this can be worse pain at night.  As SPD can stop you sleeping well and getting up to go to the toilet in the middle of the night can be especially painful.

So it is important to seek help promptly.  If possible and most Doctors would agree with this before, trying to get pregnant again. It is better for you, if you allow the symptoms from the previous pregnancy to settle.

How is SPD treated?

SPD thankfully is becoming more widely understood by Doctors, and Midwives.  It is one of those things that for some reason was not talked about and should be, from when young girls are taking sex lessons in school, to when a women is preparing to have a baby,

A woman needs to be prepared, and ladies I can’t emphasise enough the need for Pelvic Floor Exercise, you should be doing this from when you start puberty get those muscles strengthened, they do a lot of work and it can save you problems during pregnancy, also after even into the menopause .

I know a lot of you say I can’t do them or you don’t think they are that important, but they are.

To do pelvic floor and tummy exercises. Get down onto your hands and knees and level your back so that it is roughly flat. Breathe in and then as you breathe out, squeeze in your pelvic floor muscles and pull your belly button in and up. Hold this contraction for between five and 10 seconds, breathing through it. Relax your muscles slowly at the end of the exercise.

Your doctor or Midwife can refer you to a specialist women’s health Physiotherapist who should have experience in treating SPD also these days some Sports Injury Therapist that have dealt with a lot of back, hip injuries can be of a great assistance.

They will test the stability, movement and pain in your pelvic joints and muscles, they also want to hear you describe your symptoms.

Exercises especially focused on your tummy and pelvic floor muscles will improve the stability of your pelvis and back.

You may need gentle, hands-on treatment of your hip, back or pelvis to correct stiffness or imbalance. This is where a Sports Injury Therapist can be of more help to you.

Exercise in water can sometimes help, but not any strong swimming, or standard water aerobic lessons. Avoid swimming breaststroke for any length of time it’s all about gentle movements, take care with other strokes. butterfly may not be advisable

You may feel swimming is helping your pain while you are in the water, but it could make you feel worse when you get out. , talk to an instructor’ they do have special classes for pregnant ladies and knowledge of this condition is important, so be sure that they know about it.

Your midwife will also be able to give advice on how to make daily activities less painful, also on how to make the birth of your baby easier. Your midwife should help you to write a birth plan which takes into account your SPD symptoms.

Make sure any therapist is trained and experienced in working with pregnant women you may need to go the route of. Osteopathy and Chiropractic treatment, but again, see a registered practitioner who is experienced in treating pregnant women

A pelvic support belt will give quick relief, but be sure it is the right fitting for you and you are comfortable while wearing it.

Here are some Self-help tips you can follow and you can do yourself to ease your pain:

Do the pelvic floor and tummy exercises

Try not to move your legs apart when your back is slumped while you are lying down.

Take care when getting in and out of the car, bed or bath.

If you are lying down, try pulling your knees up as far as you can as it  stops your pelvis from moving and makes it easier to part your legs.

When you are sitting, try arching your back, breathing in sticking your chest out, clenching the glutes, before parting or moving your legs.

When getting dressed it is better if you sit down to pull on your knickers or trousers.

Do Not Push Through Any Pain. If something hurts,, don’t do it. If the pain is allowed to flare up, it can take a long time to settle down again. So stop, relaxes and take your time.

Move little and often. You may not feel the effects of what you are doing until later in the day or after you have gone to bed.

You must rest regularly by sitting on a birth ball or by getting down on your hands and knees. This will take the weight of your baby off your pelvis and holds it in a stable position

Choosing a practitioner

It is important to choose a Qualified Sports Injury Therapist or Qualified Massage Therapist, that has undertaken all the necessary training, to understand the theory and practice of Injury Therapy or Massage Therapy .

When I was seeing the physiotherapist from the NHS, I did not question, they were the Physiotherapist, but when it was the end of my allotted treatment entitlement.

I was still suffering and I had not progressed I knew I needed help, but that question of who do you ask came about. I was lucky, in the fact that my friend recommended a therapist that treated all the dancers at a dance club she went to , otherwise I would have been lost .

When you’re in pain and need help, you get frustrated and everything becomes a chore. You just want someone to take away the pain and sort you out with the least fuss and messing about as possible.

The first consultation  is to establish the way forward and to astatine any facts that are relevant to your problem, normally when I go to the Doctor it is a quick in and out and direct to the point, no deviations , let’s face it they don’t have that much time and they are under so much pressure.

So sitting and chatting for ½ an hour seemed a waste of time at first then I realised, it was not , so much was established and by the end of the session I could see that I could progress and we had put together a plan of action.

My questions were answered about insurance and if the therapist knew what they were doing, I felt comfortable I knew he was insured, how long he had been established.

The training he had done, the organisations he was associated with and that I was seeing the right person, yes it was confidential and treatment began.

It is important to anyone these days that if you are having treatment for rehabilitation after injury or you suffer from and ongoing condition such as arthritis or any muscular skeletal problem or just  needing a massage, be it Aromatherapy, Hot stones or Deep tissue Sports massage, in my case I needed Neuromuscular Manipulation  and Injury Therapy, you need to get The right therapist for the job.

Your therapist will get to know you, they will know what contraindications to look for during your treatment, they can advise you of these things.

That’s why a One –To- One is so important. It is this sort of things that they are taught during their course; There are, some great therapists out there, with some good qualifications and it is so important to know your therapist and trust them.

Please don’t risk going to anyone who has not got any as they may also not have the insurance either.

First  I would like to point out that the name may imply that only Tennis players got it well I am sure you all know that is not the case it does have a medical  name ,but do we all want to go round calling it Lateral Epicondylitis. This is because the pain usually occurs on the bony lump on the outside of the elbow, known as the lateral epicondyle.

Tennis elbow is fairly uncommon. Approximately five in every 1,000 adults in the UK are affected by the condition each year. Tennis elbow usually occurs in adults. Men and woman are affected equally. Well at least some things are equal even if it’s not the good things the condition tends to affect people who are around 40 years old. Are we seeing a pattern, in the fact that wear and tear comes back to hit us after the age of forty

Pain can also occur on the inner side of the elbow, which is known as golfer’s elbow. Those golfers have to get in on the act somewhere,

Now for the science the elbow joint is surrounded by muscles that move the elbow, wrist and fingers. The tendons in your elbow join the bones and muscles together and control the muscles of your forearm around the lateral epicondyle. Are you still with me on this?

When a person gets tennis elbow, one or more of the tendons in their elbow becomes. Painful. Swollen and tight the pain occurs at the point where the tendons of the forearm muscle attach to the bone.

Twisting movements, such as turning a door handle or opening the lid of a jar, are particularly painful. In other words your arm aches so much that you don’t want to move it.

Normal Outlook

Anti-inflammatory painkillers can often reduce mild pain that is caused by tennis elbow. In affect they mask the pain for a short time. Occasionally, surgery may be used to treat very severe and persistent cases of tennis elbow. we hope you come to us before it gets to that state.

Cases of tennis elbow last between six months and two years. However, in around 9 out of 10 cases, a full recovery may be made within one year. Obesity increases the likelihood of various diseases, particularly heart diseasetype 2 diabetesobstructive sleep apnoea, certain types of cancer, and osteoarthritis.

Obesity is most commonly caused by a combination of excessive food energy intake, lack of physical activity, and genetic susceptibility, although a few cases are caused primarily by genesendocrine disorders.

Our nation has now according to the news become all overweight , this in the time of the Olympics so it  may not  seem  a good image to portray, and what in truth is there to help people,

One thing I will say is don’t go extreme, anyone who has gained  weight does it over a period of time and it will also take a period of time to get rid of it. Dieting and physical exercise are the mainstays of treatment for obesity.

Diet quality can be improved by reducing the consumption, of energy-dense foods such as those high in fat and sugars, and by increasing the intake of dietary fibre.

When you do take up exercise you must be careful, start slow,with short walks, swimming or water aerobics you need to do low weight bearing exercise, water is a great medium for this there are (small local swimming clubs or see if your pool has adult times).

Also you must realise that weight, that is excess, puts a strain on all parts of your body, heart, lungs and your muscles that is a problem when you are exercising, because your body can then hold you back, muscles can strain more easily and ruptures can occur.  

So at all times take it slow in your time at your pace everyone is different and we all need to know how to work with our bodies we are after all are re-educating them.

Please remember before you start any exercise regime or diet (though a diet is the last thing you want to be doing, you should be considering it as a change of habit and a change of life) Consult your Doctor,

What not to do.

A young lady was trying to lose weight, she went on a what I call a crash diet at the same time going to the gym 4 times in a week she had decided to do a spinning class, but had not told the trainer that she had a slight pull on her thigh muscle.

She was also not drinking enough water this is important as you need to keep hydrated doing exercise,  towards the end of the class she felt unwell by the end of the night her legs were very swollen and she could hardly move she was in pain.

She had what is called Rhabdomyolysis, what we call Muscle Breakdown, she needed to have surgery to remove the damaged  toxic muscle, she was for quite sometime in hospital, it could have been even worse she could have ended up on dialysis so please take it slow, take on board the healthy eating tips.

TAKE YOUR TIME .you have nothing to prove to anyone, you can only do this for yourself.

In Pain for Months, Helped Back By Getfitstayfit Norfolk.

I was born with scoliosis and had a Spinal Fusion at three years old. The operation worked very well up until the age of around 20 when I started to notice lower back pain. This increased over the years leaving me struggling with chronic pain. I tried various things like epidural and nerve root injections but nothing seemed to work. In June 2016 I underwent surgery to correct the curve in my spine, taking a piece out and replacing it with a metal cage as well as two metal rods either side for support. Unfortunately during the operation The surgeon caught my spinal cord leaving me in Spinal Shock and temporally paralysed from the waist down, but within days I started being able to move my legs very slightly but I was unable to move my toes or my feet.

Then with a trapped nerve  somewhere in my shoulder. That I  have received treatment from a physiotherapist on the NHS      for , but sadly no response to the treatment, or indeed a diagnosis.

The pain which spread from the shoulder/neck into my head, arm, chest and back. and with the problem in my foot I was getting worn out.

Recovery and Rehabilitation after Knee Surgery

I have recently undergone knee surgery on my meniscus.

Prior to my operation Steve successfully diagnosed me with a meniscus tear, this is after my doctor told me I had an MCL tear. Steve suggested I went back to see my doctor again and asked for her to look again, Steve was correct my doctor told me that in fact I did have a Meniscus tear.

All thoughts of me carrying on my normal life were in question ( I am a martial arts instructor and also dance for a premier dance event in Norwich, Pams house).

With the aid Steve using Neuromuscular Injury Therapy along with  Bioneuro Sigma Q(ΣQ)® I have continued my life as normal attending the gym, push-bike riding, martial arts, etc. I have to credit Steve and the work he has done to keep me going, to the point that I was at the gym on the day of surgery 2 hours before. 

My surgeon told me before the operation my knee looks in excellent condition, all ligaments and muscle structure were perfectafter the operation again my surgeon visited me and told me ,

“The physio you have be having before this operation has set you up for a very quick recovery, I am very pleased and it looks like all of your hard work along side your physio (Steve) has paid off you will heal much quicker now.”

I am now 2 weeks removed from surgery and again am very much looking forward to returning to my normal active lifestyle, with Steve’s support I know I will.

In closing I would like to mention that the physio offered to me by the NHS told me  that “If you are  seeing Steve then he will be able to offer you much better rehabilitation than we ever could.” What more could I want

I so happy I found Steve and Getfitstayfit Norfolk. Now I can look forward to a successful return to full fitness.

Brian is a great guy he has been athletic all his life doing weights, swimming and water skiing and many more activities to keep fit.

His age has not slowed him down or taken away his drive to stay fit, he has not had any real problems with his joints, he is the kind of guy who hates for things to slow him down, so you can imagine how he felt when he needed to be on top form and  he sustained a shoulder injury, so was not able to be at his best.

Brian had damaged his Rotator Cuff in  his  shoulder, after a week of being in pain and finding his movement becoming more and more  restricted he went to his Doctor to see what could be done , the Doctor recommended a Cortisone injection , Brian was hesitant as it is an invasive procedure and can be painful in its self.

His Doctor told him to think about it, but booked him in for the injection for the following week, during this thinking time, he met up with Steve and told him about what had happened, Steve explained how he may be able to help and could see him later that day, encouraged by his closest friend Betty, to try something other than the injection Brian came to see Steve.

 He was put on the Bioneuro Dry System Sigma Q (∑Q®) that was able to get into the injured area and stimulate and aid in the healing  process  immediately,  the result was quite impressive, Brian felt the difference  straight away he went to see his Doctor to show him the results and to cancel the injection.

“Abuse of painkillers is putting the careers and long-term health of international footballers in jeopardy” say’s Fifa’s chief medical officer.

Dr Jiri Dvorak found that almost 40% of players at the 2010 World Cup were taking pain medication prior to every game. Ahead of Euro 2012, Dr Dvorak has urged football to wake up to the problem.

He told the BBC that younger players are imitating the seniors and taking painkillers far too frequently. Fifa’s medical team asked team doctors to provide a list of medications that players were taking ahead of each game in the 2010 World Cup.

Previous surveys at international tournaments established that many players were using large numbers of pain killing and non steroidal anti inflammatory drugs (nsaids).

“I think we can use the word abuse – because the dimension is just too much,” Dr Dvorak told the BBC.

Experts say that painkilling medication can be particularly dangerous in professional sport. In high-intensity exercise like football, a player’s kidneys are continuously working hard, making them more vulnerable to damage from strong drugs.

Dr Dvorak believes that a major factor in the growing use of painkillers in football is the pressure on team doctors to get injured players back on the pitch quickly.

“Unfortunately, there is the trend to increase the intake of medication. It is something that we have to really take seriously and ask what is behind it?”

Own Goals can be Painful

“The team doctors, most of them they are under pressure between the diagnosis and the appropriate treatment between the pressure to bring the player on the pitch, if they take them too long out they might be out of a job.”

This is  the report on Painkilling Drugs a worry to everyone, as they are prescribed on a regular basis to people who use them to MASK PAIN not to DEAL with it  and if you continue to use them they can be bad for you.

Yes there is a need for Pain Killing drugs that is not disputed, but if it is a Musculoskeletal problem that can be dealt with through Neuromuscular Release Techniques Sports Injury Therapy or Sports Massage  can help, then surely it is a better way to go.
